[This is obsolete as of February 8, 2018] All claims must be RECEIVED at the Office of the Assessor by February 8, 2018, at the latest


The Merlo Davidson Settlement Agreement stipulates that claims will not be accepted after the claims deadline which is February 8, 2018. A claim which is postmarked before the deadline but not received will not be accepted.

Claimants who are waiting for supporting documentation may still forward their completed claim forms to the Office of the Independent Assessor to ensure that the claim is received on or before February 8, 2018.

Claimants have 60 days from the day their claim is received to forward any supporting documents.  Please make a note on the claim form if you intend to forward additional information.

Claimants must include:

  • A completed claim form including a witnessed Declaration AND a witnessed Authorization and Direction to Disclose Information;
  • A completed and witnessed Certification of No Prior Compensation (available on our website); and
  • A copy of government-issued photo identification.

Claim forms can be completed and submitted (a) electronically through our secure online application process; (b) by uploading an electronic version on our website after creating an online account; or (c) by delivering a hardcopy by hand, via mail or courier.

Since our email is not encrypted we do not recommend sending confidential information by email.
